Responsibilities: Engages client in care management services; completes assessments and approves applications for services and entitlements; represents MHA and attends meetings in the community as needed; participates in care reviews, staff development programs and attends scheduled staff meetings; maintains accurate and timely documentation to effectively complete varied assignments within established time frames; with client’s input, creates individualized plans of care; consults with team members and other specialists; prepares crisis intervention plans and provide crisis intervention as required; assists in discharge planning during a client’s hospitalization and provides follow-up care as needed; assists clients and family members with concern and empathy to support treatment; and performs additional related duties as assigned.
Requirements: A Bachelor’s degree in social work, psychology, rehabilitation, nursing or a licensed health care professional with two (2) years related experience or a Bachelor’s degree or higher with three (3) related experience or a Master’s degree in social work, psychology, rehabilitation or nursing with one (1) year related experience. Experience working with a diverse population. Bilingual-Spanish or Creole a plus. Valid driver’s license
