Program  Empowerment Services and Peer Supports
Health and Community Based Services/HCBS    

Responsibilities:  Foster linkages with entitlements and community resources, model recovery lifestyle and provide companionship post-discharge from hospital or emergency department; provide companionship when an individual is in the emergency room or crisis unit; offer coaching in recovery to the individual and family, assist individual in using self-directed recovery tools, and related duties as assigned.

Requirements:  Certified Peer Specialist, as per OMH or OASAS guidelines. High school diploma or equivalent and 1-3 years of relevant experience working with individuals with substance use disorders and/or serious mental illness or a Bachelor’s Degree. Strong written, verbal, and organizational skills needed. Valid driver’s license.
