Assertive Community Treatment (ACT)

Responsibilities:   Dual position working in PROS and ACT.  In PROS, work in the medical services office with adults recovering from mental illness.  Specific responsibilities include but are not limited to:  scheduling appointments; preparing and recording Rx’s; checking in and dispensing medication; assessing and acting upon immediate medical problems; obtaining releases and sending report forms to physicians when due; assisting in health education and teaching; and maintaining current medical information on consumers.  In ACT, be part of a mental health team that delivers services to individuals in their homes/communities. Services provided are geared toward satisfaction of daily living responsibilities, helping individuals develop the tools to obtain and maintain housing, employment, relationships, and relief from symptoms.  Conduct physical assessments; determine physical health needs; refer to community physicians for needed medical treatment; assist the ACT Psychiatrist; manage and administer medication and provide a range of direct treatment, support, and rehabilitation services to program consumers.  Provide service coordination and chart documentation, attend daily team meetings, participate in the development of treatment plans, record treatment interventions with consumers, participate in and initiate training, monitor medication side-effects and perform additional duties as assigned.

Requirements: Completed RN nursing program accredited by the NYS Department of Education.  Must be licensed or eligible for NYS licensure.  A valid driver’s license and geographical knowledge of Rockland County.  Experience working with adults with severe mental illness.  Experience working with a diverse population preferred. Covid-19 vaccination preferred.
