Licensed Professional Clinician II

35 hours/week, benefits eligible

Program:    Mobile Outreach Team (MOT)

Responsibilities:  Supervises the day-to-day functioning of the MOT.  Responsibilities include but are not limited to overseeing the scheduling of visits; ensuring that all documentation is completed in a timely manner; providing supervision and direct service, which will include engagement, assessment, accompanying the individual on behavioral health and medical appointments, supporting residential staff and family, working with hospital staff on discharge planning, and assisting in crisis prevention and response.

Requirements:   Licensed practitioner in mental health field, (LMSW, LMHC, etc.) and 2 years of experience working with individuals recovering from significant mental health issues and those with co-occurring disorders. Supervisory experience preferred. Experience working with individuals with mental illness in a both group and individual settings. Ability to engage clients through the development of a therapeutic relationship; to deal with crisis situations and adapt to new or changing conditions and to work as a team and participate in a group process of goal setting and problem solving.  Excellent verbal and written communication skills.  Deep understanding of recovery process and principles of person-centered planning.  Comfortable going into the community, meeting individuals on inpatient units and in residences. Knowledge of community resources and mental health services.  Proficiency with Electronic Health Records preferred.  Experience working with a diverse population.  Valid driver’s license and ability to drive program vehicles.
